Registration

I hereby register my child at Southwest Dance Company for the 2024-2025 school year. Tuition is due by the 5th of each month. A $10 late fee will be added after the 10th of each month.
In case of suit or action is instituted to collect tuition or any other fees due to Southwest Dance Company, the below named promises to pay all collection costs and such additional sums as the court may adjudge reasonable such as court costs, attorney fees, service of process, etc. in said suit or action.
I agree to pay an annual family registration fee of $45.00 per family per year due upon registration. If my child discontinues classes at Southwest Dance Company for any reason, the registration fee will be forfeited. There are no credits or refunds for lessons missed. A written 30 day notice is required to drop a class or tuition will continue to be billed to your student's account.
I agree to pay the returned check fee of $30.00 for any check returned to the bank. I understand that pictures taken of my child(ren) while at Southwest Dance Company are the property of Southwest Dance Company and may be used for promotional advertising.
RELEASE: I hereby release Southwest Dance Company and all staff officials from all claims of damages or injury suffered by the registered student in connection with their association with Southwest Dance Company. This includes my heirs who may not act in my behalf. I have read and understand the terms of the above contract and the program information sheet and agree to abide fully by its terms.
